A Form Report displays records submitted through an approval form. With AI-assisted creation, you can describe which form to use and what information the report should show, then review the proposed configuration before creating it.
For example, you can create a purchase request report to help your team review submitted requests, track approval status, and find relevant records.
Before You Start
Open the application where you want to create the Form Report. Make sure you have permission to add application resources and access the source approval form.
Before starting, identify:
The approval form the report should use.
The form fields or variables you want to display.
Any conditions that should limit the records included.
A Form Report uses an existing approval form as its source. It does not create a new approval form or submit new records.
Step 1: Open New Application Resource
In your application, click Add resource (+) to open New application resource.
In the Create with AI area, select Form report as the resource type.
Step 2: Describe Your Report Requirements
Enter a description of the report you want to create.
Specify the source approval form by name and explain which information the report should display. Use the actual field or variable names where possible.
For example:
Create a Form Report named Purchase Request Report using the existing Purchase Request approval form. Display Request Number, Applicant, Department, Request Date, Total Amount, and Approval Status. Include only requests where Total Amount is greater than 10,000.
The names in this example are illustrative. Replace them with the names used in your application.
For better results, include the following details:
Detail | What to include |
Report name | A clear name, such as Purchase Request Report |
Source approval form | The existing approval form the report should use |
Displayed information | Fields or variables to include |
Record conditions | Rules that determine which records appear |
Business purpose | Who will use the report and what they need to review |
Click Start with AI to submit your request.
Step 3: Review the Implementation Plan
AI analyzes your requirements and prepares an Implementation Plan.
Review the proposed configuration carefully, particularly the source form and the mapping of form information to report fields.
Review area | What to check |
Report identity | The name and purpose match your requirements |
Source form | The correct approval form is selected |
Report fields | The required information is included |
Field mapping | Each report field refers to the correct source information |
Record conditions | Filters include the intended records |
If the source form or any field is unclear, clarify it in the conversation before confirming creation.
For example:
Use the Purchase Request form in the Procurement Management application. For Total Amount, use the form variable named RequestTotal.
Or:
Remove the amount condition so the report includes all purchase requests.
Review the revised plan before proceeding.
The Form Report is created only after you confirm the Implementation Plan.
Step 4: Confirm and Create the Report
When the plan meets your requirements, confirm it in the AI conversation.
AI creates the Form Report based on the confirmed plan. Follow any additional setup prompts shown in the interface.
When creation is complete, open the report using the resource link provided in the conversation.
Step 5: Check the Created Report
Open the Form Report and review its configuration and displayed records.
Check that:
The report uses the correct approval form.
The expected report fields are available.
Field values match the corresponding source records.
Record conditions work as intended.
The report name and field labels are clear.
If the source form already contains records, compare a few report entries with their original submissions.
Creating a Form Report does not generate submission records. If the report is empty, check whether the source form contains records and whether those records match the report’s filters.
Step 6: Review Views and Permissions
Before sharing the report, review its data views and access permissions.
Configure the views, displayed columns, sorting, and search options needed by your users. Check who can access the report and which operations they are permitted to perform.
Do not use report filters as a substitute for access permissions. Verify the permission settings separately before making the report available.
Continue Refining with Ask AI
After creation, use Ask AI to return to the original saved conversation and request further changes.
For example:
Add the Expected Delivery Date field from the source approval form to the report.
Or:
Update the report to include only requests from the Operations department.
Review the proposed changes and verify the resulting report configuration.
Tips for Better Results
Identify the Source Form Clearly
Use the exact approval form name. If similar names exist, include the application name to help distinguish them.
Use Existing Field Names
Refer to fields or variables that already exist in the source form. If you need information that the form does not collect, update the source form separately.
Write Precise Filter Conditions
Specify the field, operator, and value.
For example:
Include requests where Total Amount is greater than 10,000 and Department equals Operations.
Distinguish Reporting from Workflow Changes
A Form Report presents approval form records. If you need to change approval routing, task recipients, or workflow actions, edit the source workflow separately.
Verify the Result with Actual Records
Check a small selection of submissions to confirm that field mappings and filters produce the expected results.
Frequently Asked Questions
Does AI create the report immediately after I submit my request?
No. AI first prepares an Implementation Plan. Review and confirm the plan before the report is created.
Do I need an existing approval form?
Yes. A Form Report uses an existing approval form as its source.
Can I change the proposed fields before creation?
Yes. Explain the changes in the conversation and review the revised plan before confirming it.
Does creating a Form Report change the source approval workflow?
Creating the report does not change the source workflow’s approval routing or actions. Configure workflow changes separately in the workflow designer.
Why is the report empty?
Check whether the correct source form is selected, whether it contains submission records, and whether the report’s filters exclude those records. Also verify your access permissions.
Can I continue modifying the report after creation?
Yes. Use Ask AI to return to the saved conversation and describe the changes you need. Review the updated configuration and displayed records afterward.
